Please fill out the form below to request a contact lens fitting with your doctor. At this visit, your doctor will assess the lens fit, movement and vision. We will make the necessary modifications to improve the vision and/or comfort of your lenses. There is a $50 fee for a single vision contact lens fitting and a  $70 fee for monovision/multifocal contact lens fitting. Free contact lens trials and follow up appointments are included in this fee.
